CANCELLATION / ENROLLMENT TERMINATION POLICY
Your child is automatically re-enrolled at My Gym until an official Cancellation Request Form (CRF) has been completed and submitted to us. The CRF is available for download on this page (see above) or at the Gym. It is agreed that in order to terminate your enrollment, a CRF must be received at least twenty one (21) days prior to next billing date. Failure to meet that requirement will result in the member being responsible for one final payment. This agreement will then terminate four (4) weeks from that last scheduled payment. Children may continue to attend classes and Free Plays as well as complete any remaining make-ups through that date. Any remaining make-up classes after the last class date will be forfeited.
MAKE-UP POLICY (INCLUDING INCLEMENT WEATHER)
Classes missed for any reason, may be made up by attending any age-appropriate class that we offer, subject to space availability. Make-ups must be taken while the student is enrolled in a class. Make-ups are transferrable to Sibling(s) within the same household. Those make-ups must be taken in any age-appropriate class, subject to space availablity. Unused make-ups may not be carried forward. A refund or credit will not be issued for missed classes. My Gym does not pro-rate class tuition.
In the case of snow, ice or severe weather, My Gym may decide to cancel classes for that day or for a part of that day, based on the conditions of the surrounding area. If in doubt, please call 847-229-1990 for information. We will leave cancellation information for that day on our voice mail by 8:30 AM. Also, please check the main home page of this web site for closure information. If you see none, assume that we are open. Missed make-up classes are forfeited.
REFUND POLICY
We do not grant credit or issue refunds for missed classes. We encourage you to make up any missed classes due to illness, vacations, school activities, inclement weather or school closings. Please see our make-up policy (above) for information on rescheduling a missed class. A refund or credit will only be considered in the case of long-term illness or injury, or an unexpected relocation. A 20% Refund fee is applied to all refunds approved by My Gym. There is no Refund fee for refunds issued due to classes or camps cancelled by My Gym. There are no fees for Store Credits.
Birthday Party Deposits are non-refundable. If you need to switch dates/times, you will be required to put down another non-refundable deposit. We will make your original date available to the general public and when/if it re-books, your original deposit will be credited to your My Gym account as Store Credit ONLY and will be available to you immediately.
Membership fees are non-refundable. |
